RESUMO

BACKGROUND: Head and neck mucosal melanomas (MMs) are rare tumors with adverse outcomes and poorer prognoses than their more common cutaneous counterparts (cutaneous melanomas-CMs). Few studies have compared the expression of mitochondrial dynamic markers in these tumors. This study aimed to assess the correlations of mitochondrial markers with melanoma progression and their potential as predictors of lymph node involvement and distant metastasis. METHODS: Immunohistochemistry against anti-mitochondrial (AMT), dynamin-related protein 1 (DRP1), mitochondrial fission protein 1 (FIS1), mitofusin-1 (MFN1), and mitofusin-2 (MFN2) antibodies was performed in 112 cases of head and neck CM and MM. A Cox regression multivariate model was used to assess the correlation of AMT, FIS1, and MFN2 expressions considering the risk for nodal and distant metastasis. RESULTS: All markers studied presented higher staining in tumor cells than normal adjacent tissues. Higher mitochondrial content was observed in MM than in CM, and it was significantly associated with nodal metastasis in oral melanomas. Both FIS1 and DRP1 expressions were related to advanced Clark's levels in CM, and they were overexpressed in oral melanomas. Moreover, increased immunoexpression of MFN2 was significantly associated with a higher risk of metastasis in CM, and it was also overexpressed in sinonasal melanomas. CONCLUSIONS: Our results suggest that mitochondrial fission and fusion processes can play an important role during multiple stages of tumorigenesis and the development of nodal and distant metastasis in cutaneous and mucosal melanomas.

RESUMO

Surgery is the most effective way of treating a locally advanced colorectal carcinoma and an extended en bloc resection is necessary to achieve the best overall survival rate. In this rare case, a multi-visceral resection was performed along with the entire lower left limb and left iliac bone for a sigmoid carcinoma. The T4N0M0 (B3) tumor involved the left iliac vessels, left pelvic wall, small bowel and both rectus muscles, besides presenting with a skin fistula. A Hartmann colostomy was also performed. Chemotherapy was interrupted because of toxicity. The patient is free of disease after 38 months. There are very few cases that describe an extended hemipelvectomy as part of a colorectal carcinoma treatment.

Dois casos de paracoccidioidomicose genital säo relatados. Esta forma rara de doença é apresentada em pacientes com comprometimento pulmonar prévio e que queixavam-se de aumento doloroso de testículo que comprovou-se conter P. brasiliensis. Ambos os casos nos atentaram para a possibilidade dessa täo rara apresentaçäo de blastomicose sul-americana

RESUMO

Sessenta pacientes consecutivos foram submetidos à cirurgia de vias biliares, através de mini-laparotomia no período entre agosto de 1991 e março de 1993. A idade dos pacientes variou de 17 a 75 anos, com média de 47 anos, sendo 08 (13,3 por cento ) do sexo masculino e 52 (86,6 por cento ) do sexo feminino. A maioria das cirurgias, 53 (88,3 por cento ), foram realizadas eletivamente. Cinquenta pacientes eram portadores de colecistopatia calculosa (83,3 por cento ) sendo que uma apresentava concomitantemente carcinoma avançado de vesícula biliar; 7 (11,6 por cento ) pacientes com colecistite aguda; e 2 (3,3 por cento ) com colecistite crônica com coledolitíase; e um portador de pólipos intra-vesicular (1,6 por cento ). Em todos os pacientes realizamos colecistectomia através de incisöes que variaram de 4,5 a 10 cm (média de 7,02) tendo sido realizado colangiografia per operatória em 25 (41,6 por cento ) com tempo cirúrgico médio de 52 minutos (de 25 a 70). A grande maioria obteve alta nas primeiras 48 horas, com baixo índice de morbidade, havendo apenas um óbito (1,6 por cento )
